Wat darmintegriteit betekent voor auto-immuunziekten

Auto-immuunziekten, met hun scala aan klachten, vormen een wereldwijd probleem. Steeds meer richt onderzoek zich op de rol van de darmen in de immunologische processen die gericht zijn tegen eigen weefsel. De darm fungeert als barrière en beslist daarmee over de passage van stoffen en het al dan niet in gang zetten van een immuunreactie; essentieel voor behoud van homeostase. Wat betekent darmintegriteit voor de auto-immuunaandoeningen reumatoïde artritis, multiple sclerose, diabetes type I en systemische lupus erythematodes?
Auto-immuun betekent dat het immuunsysteem zich tegen het lichaam zelf keert. Het komt veelal voort uit het wegvallen van immunologische tolerantie, waardoor het immuunsysteem niet alleen maar in actie komt tegen ‘vreemd’, zoals gewenst, maar ook tegen ‘eigen’.1 De prevalentie van auto-immuunziekten neemt de laatste decennia drastisch toe en stijgt bovendien onder jongere bevolkingsgroepen.2 Het gaat gepaard met een grote sociaaleconomische last en hoge zorgkosten. Vrouwen blijken gevoeliger voor het ontwikkelen van auto-immuunziekten, vanwege hun verhoogde immuunreactiviteit. Zowel genetische als omgevingsfactoren liggen ten grondslag aan het ontstaan van auto-immuunziekten.
